REMOTE MONITORING AND PERSONALISED PLANS

The Monsenso digital health solution is a cloud-based solution for mental health support teams, enabling them to monitor the service users and provide personalised, coherent support plans – either remotely or through blended care.

It provides visualisations of service users’ data captured from their smartphone, pointing out those areas that may require support, while providing remote care to the service users to promote independence in the community.

The data captured on the individuals’ smartphones can help multidisciplinary support teams to personalise support plans further, so the right individuals receive the right treatment at the right time

The opportunity

Person-centred care planning

Central to person-centred care planning is to understand the individual’s mental wellbeing and how it affects their daily functioning. However, without insight into the individual’s daily mental wellbeing, it is difficult both to assess eligibility in the need assessment and to tailor the care and support plan to meet the individual’s needs.

Our solution

Our digital health solution enables local authorities to remotely monitor individuals collecting self-reported outcomes and sensor data on behaviour, providing insight into the individual’s day-to-day mental wellbeing and how it affects their daily living.

Treatment adherence

Medication non-adherence is unfortunately common, especially among patients with mental illness, and premature discontinuation of mental health treatment can compromise treatment outcomes and potentially worsen symptoms.

Our solution

Our solution provides service users with daily reminders to take their medication and adhere to their care plan, as well as ‘tools and strategies’ to help them learn how to manage difficult situations in their daily lives. The solution also enables immediate support to be provided remotely to those in need, promoting adherence to treatment.

Access during the pandemic

COVID-19 has had a major and sustained impact on health and social care delivery across the globe, forcing organisations to shift, almost overnight, from in-person services to digital delivery.

Our solution

Our solution enables local authorities to remotely monitor service users (safeguarding the well-being of staff) while maintaining access to care and providing immediate support for individuals in need.

Multi-disciplinary coordination

Ongoing communication and shared insight into the service users’ mental wellbeing and needs are instrumental for multidisciplinary teams (MDTs) to provide holistic, person-centred and coordinated care and support.

Our solution

Our solution enables one point of access supporting pratitioners within multi-disciplinary teams to deliver an evidence-based and coordinated mental health service to their citizens.
