The Monsenso digital health solution is a cloud-based solution for mental health support teams, enabling them to monitor the service users and provide personalised, coherent support plans – either remotely or through blended care.

Person-centred care planning

Central to person-centred care planning is to understand the individual’s mental wellbeing and how it affects their daily functioning. However, without insight into the individual’s daily mental wellbeing, it is difficult both to assess eligibility in the need assessment and to tailor the care and support plan to meet the individual’s needs.

Our digital health solution enables local authorities to remotely monitor individuals collecting self-reported outcomes and sensor data on behaviour, providing insight into the individual’s day-to-day mental wellbeing and how it affects their daily living.

TREATMENT ADHERENCE

Medication non-adherence is unfortunately common, especially among patients with mental illness, and premature discontinuation of mental health treatment can compromise treatment outcomes and potentially worsen symptoms.

Our solution provides service users with daily reminders to take their medication and adhere to their care plan, as well as ‘tools and strategies’ to help them learn how to manage difficult situations in their daily lives. The solution also enables immediate support to be provided remotely to those in need, promoting adherence to treatment.
